By subscribing, you are confirming that:

• You understand, and agree, that customer public or private data must be handled according to federal and state regulations, and to have access to this data you confirm that you have read and agree to adhere to the terms & conditions set forth in the Guest Information Access Agreement.
• I have read and agree to the terms and conditions as found on the CoGoBuzz website and confirm I am authorized to enter into an agreement and submit payment for the approved charges using the enclosed form(s).

Terms:

1. Service. This is a subscription based service. The credit card information provided below, or via phone, will be billed automatically every 30 days. Your first payment is due at the date the order is placed. Your first recurring payment will be billed 30 days from the date your package was received.
2. Termination. By subscribing, the Customer has agreed to discounted service with a 12 month service commitment. Terminating service before the end of the agreement results in a minimum payment of $25 per month for the duration of the agreement term; price subject to change. Services canceled at any time prior to the Agreement end date, require all CoGoBuzz hardware to be returned in provided packaging and in good, working order. HNG may cancel any activity without notice. The Customer is required to return any and all Unit(s), to be delivered to HNG offices, within five days of termination. In the event the Unit(s) are damaged, lost, or stolen, the Customer agrees to pay the full equipment cost of $149 as provided in the accompanying invoice. If your deposit was less than this amount, as part of a special offer, you will be billed for the difference. Service cancellation requires a 30 day written notice.
